Hope & Home Care Community advocates client independence and supports inclusion

Hope & Home Care Community is an innovative service provider of individualised support and prides itself in offering flexibility, choice and control to people with disability. It is a registered provider of National Disability Insurance Scheme (NDIS) services for those with disabilities which offers a range of services including community nursing care, support coordination, daily personal care, community participation, transitional care, transportation and domestic/household tasks. 

Hope & Home Care Community was founded by Ann Razon and Lydia John in August 2018. The company started with only one client and has since flourished to be an established and reputable company that has served for three years and counting in the NDIS community. It now boasts of numerous clients who are loyal to Hope & Home Care Community.

Moreover, the company is composed of loyal and experienced staff who explore possibilities, persevere through challenges and celebrate the wins of the clients. The employees have a range of skills that compliment the NDIS mission and have on average more than 15 years of experience in the service. 

Over the years, the company has improved the quality of service to deliver a more efficient, individualised and genuine support care to every single client to cater to their unique needs and circumstances.

The mission is to enhance the lives of people with a disability by enabling opportunities for choice, inclusion and achievement at home and in the community. Hope & Home Care Community is committed to ensuring that the participants are among those who drive change within the organisation. Furthermore, it is committed to producing measurable outcomes in a person’s quality of life, increasing opportunities for independence and choice for all, and endeavouring to strengthen clients’ relationships and their inclusion in the community. Hope & Home Care Community drives to be accountable in all of its work, responsible for its actions, embraces diversity, and is always respectful of others’ choices.
